· a disabled persons tax credit NHS exemption certificate

Check the guidelines below to see if you are eligible for a free eye examination from the NHS:

AGE:

· If you are under the age of 16

· If you are aged 60 or over

· If you are a full time student aged 16,17 or 18

HEALTH:

· If you suffer from diabetes / glaucoma

· If you are aged 40 or over and are the parent, brother, sister or child of a person with glaucoma

· If you have been prescribed with complex lenses under the NHS optical voucher scheme.

INCOME:

If you or your partner receives:

· Income Support

· Job seekers allowance

· a working families tax credit NHS exemption certificate

If you receive a low income you may be eligible to NHS help but you must complete an HC1 form, which is available in the practice.

· If you are registered blind / partially sighted
